  • referrals to Algonquins of Pikwakanagan Home Care
  • referrals to Ontario Health atHome for home nursing, * occupational therapy, * physiotherapy
  • provides patient advovcay as requested by clients for appointment, etc
  • maintains an equipment lending system for medical or equipment supplies on a short term basis for home and community care and home care clients

Eligibility - Population(s) Served: Must be a residing, registered status Algonquin of Pikwàkanagàn First Nation member * no age restriction * people living with a disability * chronic or acute illnesses * convalescing at home individuals * the elderly
Application: Referrals can be made in person, by phone or in writing by client, caregivers or other health providers
