Description of Services:	Support and services to individuals who have experienced a brain injury * the focus is on helping clients with the rehabilitation and community reintegration process, offering a range of services that can include residential programs, community-based support, and day programs Residential Programs 24/7 Care: For individuals requiring constant support Individualized Care Plans: Each resident receives a personalized care plan tailored to their specific needs, which might include medical support, life skills training, and therapeutic activities Community-Based Services Outreach Programs: allow clients to live independently or with family while receiving support such as daily living skills, community integration, and socialization Rehabilitation Support: assistance in relearning or adapting to skills necessary for daily life, such as cooking, budgeting, and using public transportation Day Programs Structured Activities: Clients can participate in day programs that offer structured activities aimed at enhancing cognitive, physical, and social skills Social Interaction: programs also provide opportunities for clients to engage with peers, reducing isolation and building a sense of community Family Support Education and Training: for family members to better understand brain injuries and how to support their loved ones effectively Counseling and Support Groups: Families can also access counseling services and support groups to share experiences and receive emotional support Advocacy and Case Management Navigating Services: Case managers work with clients to help them navigate healthcare systems, access benefits, and connect with other community resources Advocacy: on behalf of clients to ensure they receive the services and support they need from various agencies and organizations Recreational Therapy Therapeutic Activities: used to engage clients in activities that promote physical health, mental well-being, and social interaction Creative Arts: Activities such as art, music, and drama are often included to help clients express themselves and improve cognitive function Services and Programs: Adult Day Program Assisted Living Services Personal Support Independence Training

Eligibility - Population(s) Served:	Persons 18 years of age and older who have an acquired brain injury, who are medically stable and have completed post-injury rehabilitation, who do not require daily supervision, and who are willing to participate fully in the program
