Description of Services: Provides comprehensive geriatric mental health assessments to provide diagnosis and treatment options for older adults (65 years and over) living in the community with new mental health concerns, and/or chronic mental illness complicated by age related functional decline, and/or living with dementia and associated behaviour change

Outreach services provide in-home assessment, short-term case management and treatment to optimize quality of life and recover

Interdisciplinary team includes: Geriatric psychiatrists * registered nurses * occupational therapists * social workers * psycho geriatric resource consultant

Fees: Set fees, Covered by the Ontario Health Card
Eligibility - Population(s) Served: Residents of Ottawa 65 years and older with mental health problems or suffering from dementia who are not in a long-term care home, and persons less than 65 with Alzheimer's dementia or Frontotemporal Dementia complicated by behavioral or psychological symptoms
Application: All referrals need to be signed by a physician along with their billing numbers
