Description of Services: Interdisciplinary assessments, diagnostic and treatment services for geriatric patients with medically complex problems in partnership with community organizations and services to enable these individuals to remain in their homes and communities for as long as possible

Day program addressing issues such as:
  • Mobility/Falls
  • Difficulty with activities of daily living
  • Cognitive issues and symptoms
  • Medication concerns
  • Caregiver stress and future planning

Eligibility - Population(s) Served: Seniors, 65 years of age and older * younger individuals presenting with geriatric problems that may benefit from specialized geriatric services are evaluated on an individual basis * residents of Ottawa-Carleton or the surrounding areasl
