Description of Services: Comprehensive geriatric health assessment for seniors with complex health needs, in their own homes * this interdisciplinary team of professionals serves as a primary point of access for community referrals to the Regional Geriatric Program of Eastern Ontario, a network of specialized geriatric health services designed to avoid or reduce preventable losses of health and independence associated with aging

Fees: None with OHIP Card
Eligibility - Population(s) Served: Seniors 65 years and older * younger individuals presenting with geriatric problems that may benefit from specialized geriatric services are evaluated on an individual basis * residents of Ottawa-Carleton or the surrounding areas
