Description of Services: |
Integrated community mental health support services for individuals with a severe and persistent mental illness and/or concurrent disorder and those who are homeless or at risk homelessness with complex needs Services include: Intensive Case Management, Housing Support, Court Diversion/Support, Vocational Support, Health Promotion, Hoarding Support * Services are offered in the client's home and in the community Community Homes for Opportunity (CHO): a recovery-oriented person-centred supportive housing program for people with serious mental illness who may require 24 hours supportive care * The program is intended to assist tenants by providing appropriate housing and support services to achieve and maintain housing stability that is safe and affordable Resource Centres provide day programming for adults with a serious mental illness and/or concurrent disorders as well as those socially isolated Mental Health Promotion provides workshops on a variety of mental health and well-being topics Services and Programs: |
