Transportation services for older adults and people with disabilities living in the Champlain District * provided by a collaborative of 26 Community Support Service (CSS) organizations, divided into four catchment areas Medical appointments * discharge from hospital * day programs * occupational therapy (OT) assessment * daily living activities such as work, grocery shopping, banking, Diner's club, pharmacy, etc * centers need between one to seven days notice for transportation |
